Information-gathering, fact-finding - An in-depth, in-person assessment, generally with two of our nurses who come to your elderly loved one’s home to conduct that assessment.
Discussion of key considerations and options - Our nurses follow-up with research to identify options that are specifically tailored to your situation.
Preparation of a personalized report with specific resources and recommendations - Based on our findings, our nurses prepare specific, prioritized recommendations for you and your elderly loved one.
Review of findings with you and your family - We schedule a follow-up meeting with you and your elderly loved one to present our findings and, of course, to answer any questions.
